If your child is interested in participating in fall sports at FVMS, click on the following link for middle school eligibility requirements and athletic participation forms. Before a student is eligible to participate in athletics, the athletic participation form must be completed by a parent/guardian and physician. Physical examinations can be difficult to schedule at the beginning of the school year so we suggest that this step be taken as soon as possible.
