It is the policy of the Wake County Public School System to provide concussion information to student athletes and the parents/guardians of student athletes. If your child intends to participate in FVMS athletics, parents/guardians and students must sign separate concussion information forms. These forms must be signed and returned to the team coach before team tryouts begin. Tryouts begin Monday, August 29. Please click on the following links to download and print your copy.
